I agree that a program with Dara Allen Enterprises is subject to the following terms: The above named candidate (s) has my permission and consent to participate in programs at a classroom, pool facility, and or other venue. In the event of an illness or injury while participating in programs with Dara Allen Enterprises, a designated person is hereby authorized to obtain treatment by a medical profession for the candidate (s), as I would do if I were present. In consideration of the candidate(s) being able to participate in programs with Dara Allen Enterprises, I do hereby release and hold blameless Dara Allen Enterprises from all claims, liability, costs and expenses arising out of or resulting from the candidate (s) participation, or medical treatment obtained. The candidate(s) is and or are covered by a family health plan, or government medical plan. Dara Allen Enterprises does not provide medical insurance covering injuries of any nature while participating in programs. I understand my child must be covered, and hereby confirm that they are. I am aware of the risk of injury or accident that may occur while participating in the following activities: Swimming lessons, Lifesaving skills, First Aid Skills both in and out of the water. I freely provide personal information that is pertinent about my child or family for the daily operation of programs with Dara Allen Enterprises.
